Any person can tell whether a face is masculine or feminine just by looking at their facial features. This ability starts during the earliest childhood, even before learning which the genital differences are. However, most people wouldn't be able to put into words what exactly makes a face look female or male, nor have they even thought about this. The shapes and profile that we see are determined by factors hidden within: the bones constituting the forehead, nose, jaw and chin.

Although many masculine-faced women can get a satisfactory feminization through relatively conventional facial surgery, many others need to seek a change in the deepest layers of the face, altering the proportions of the facial skeleton with the best FFS. Surgery like this wasn't available until the early eighties, when Dr. Douglas Ousterhout introduced the concept that it was possible to achieve the feminization of a male face by using cranio-facial surgery techniques that were so far just intended to fix cranial malformations or injuries from accidents. Since then, the set of surgical procedures performed to reshape the facial bones and give them feminine proportions is called FFS (Facial Feminization Surgery). Can Facial Feminization Surgery be combined with a face lift?

Although some FFS surgeons prefer to perform face and neck lifts in another surgery some months later than the bony Facial Feminization, surgeons like Dr. Rossi think that both kinds of procedures can be combined in just one surgical act with very good and especially natural results. When the face lift is performed in one surgery with the feminization of the facial skeleton, it must be understood as a complementary procedure that repositions the soft tissues in a way that brings out the feminization acquired on the facial bones (that would otherwise get concealed, not fully appreciable) and as a factor that sums up into the whole facial transformation rather than as a facelift per se, usually intended for just rejuvenation. In terms of rejuvenation it might result less perfect than when performed alone, but the combination has a great benefit for the senior patients: it produces an immediate younger appearance that allows them to recover and enjoy in the right gender many years they spent living in another one.

Partial facial feminization surgery

Many MtF people don’t need to undergo a full FFS. Surgeons like Dr. Rossi know that sometimes a surgery of just the forehead and the nose can be enough to produce a satisfactory degree of feminization. This is because the relative importance of the forehead in the perception of the facial gender is much higher than the relative importance of the lower face, as the forehead affects something crucial: it affects the way the eyes look. It is amazing how something as abstract as the look of the eyes can be changed by reshaping hard bone. A male bone frame around the eyes gives the look the fierce, severe or even grim character of the predators, probably an heritage from the age when male were just hunters and fighters. The best FFS surgeons can tell when less is more. Many times a surgery of the forehead and the nose gives the eyes of the MtF patients the look of the female eyes, and this is all that was needed. This look, so appreciated by romantic poets, is not female because of anything in the eyes themselves (which are the same eyeballs in both males and females), but because of the shape, thickness and especially the projection of the bones that surround them.

When brow bossing protrudes it results the most telltale of male facial areas. Most of times it must be set back through an MtF surgery called forehead reconstruction to feminize it properly. Only the best FFS surgeons can perform it. Thick orbital rims give the eyes a male look. Their feminization is achieved through grinding/burring them down (shaving).
