Forehead contouring is the most impactful single procedure in facial feminization — addressing the brow ridge, frontal bone and hairline to create a smooth, feminine forehead profile.

What is it

The forehead is the strongest gender marker in the face

Most people don't consciously notice it — but the shape of the forehead is one of the clearest anatomical gender markers. A female forehead has a smooth, gently curved surface and draws a near-vertical line on the facial profile. A male forehead, by contrast, has a pronounced brow ridge (brow bossing) thicker and lower-sitting orbital rims and a more sloped profile. And these skeletal differences determine in turn the most impactful difference of all — the gender of the gaze.

The brow bossing is a bony shell covering a hollow cavity — the frontal sinus — located just above the nose. This is key to understanding why forehead feminization requires more than simple shaving in the vast majority of patients.

The male hairline also tends to form an "M" shape, while a feminine hairline curves in a smooth arch, closer in shape to a in "inverted U". Forehead contouring surgery can address all of these differences in a single procedure.

Which technique is right for you

The choice of technique depends on the thickness of the anterior wall of the frontal sinus — determined during your pre-surgical consultation and CT scan review.

For ~5% of patients

Forehead Shaving
(Burring / Grinding)

When the anterior wall of the frontal sinus is thicker than the amount of the setback desired (or when the frontal sinusis nonexistent), the brow ridge can be ground down directly — without opening the sinus. This is only suitable for a small minority of patients whose bone structure allows sufficient reduction by shaving alone.

Forehead Reconstruction
(Type 3 Procedure)

The anterior wall of the frontal sinus is carefully cut with an ultrasound saw (piezotome) and removed, reshaped and repositioned — acting as a bony cap that creates a new, feminine forehead contour. This is the gold standard and the most common technique, as most patients don't have enough bone thickness for shaving alone to achieve true feminization.

About the incision

The incision, which runs from ear to ear, can be done along the hairline just in front of it (trichophytic incision) or hidden in the hair, depending on whether the patient wants to lower the hairline or not. Hairline indision allows for hairline lowering and forehead shortening, while the coronal incision (hidden in the hair) doesn't. If the hairline has an acceptable height in its middle portion, it's best to do the incision hidden in the hair and use hair transplants to fill in and round out the receded corners. These hair transplants can be done in the same medical trip, under local anesthesia, 2 weeks after the FFS, but only if the incision is done in the hair. If the incision is done pre hairline, then hair transplants must be done any time later, once the scar is healed.

  • Brow Lift — elevating and arching the eyebrows to a feminine position, through the same incision
  • Hairline Advancement — softening the M-shaped male hairline and moving the scalp forward up to 2cm when needed
  • Rhinoplasty — feminizing the nose in the same session for a harmonious upper and mid-face result. Many times the top of the nose bridge must be pushed back at the same time as the forehead for both areas to match, even if a rhinplasty has been performed before
