The key insight in jaw feminization is that the goal is not simply to make the jaw angles look less angular — it is to make the entire facial frame look narrower from the front. Many women have defined jaw angles, yet their faces read as feminine because the overall width is narrower. That is exactly what this surgery achieves.
The surgeon accesses the jaw bone through an incision inside the lower gums — leaving no visible external scars. The bone ridge along the lower jaw is reduced with precision drills, and the rear angles are trimmed and rounded. This tapers the sides of the jaw and reduces its height, creating a softer silhouette.
When the masseter muscles are excessively developed, they can also be surgically reduced — though in many patients, sustained hormone therapy produces a comparable reduction over time. Jaw feminization is almost always combined with chin feminization for a harmonious lower face result.

Jaw and chin are anatomically connected and visually inseparable. Contouring the jaw without addressing the chin often produces a result that looks incomplete or imbalanced. For this reason, the vast majority of jaw feminization procedures are performed together with chin reduction and reshaping in the same surgical session.
For patients over 40, it is also worth considering a face lift and/or neck lift to tighten loose skin and allow the newly feminized bone structure to show through — otherwise, the results can remain partially concealed by excess skin.
Possible risks
Asymmetry
Even if the area is carefully measured and marked before surgery, asymmetric scar tissue during healing can cause slight asymmetries.
Temporary changes to nerve sensitivity
Lingering numbness or tingling in the lower face — resolves as the nerves become less inflammed over weeks to months.
Infection
Very uncommon when antibiotics and oral hygiene protocols are properly followed.
