The eyebrows frame the eyes — and often, they can be a gender marker.
The eyebrows are one of the most emotionally expressive features of the face — but they can also be an anatomical gender marker. Female eyebrows tend to be curved and sit above the orbital rim, creating an open, soft eye area. Male eyebrows, by contrast, tend to be straighter and lower — often below the rim — giving the eyes a heavier, more severe appearance.
The position of the eyebrows is directly determined by the shape of the underlying frontal bone. This is why brow lift is almost always performed at the same time as forehead contouring — the bone is reshaped first, and the eyebrows are then repositioned to sit naturally in their new feminine position. Without the brow lift, the soft tissue would simply fall back into its original position or even sag.
When performed alone, the brow lift is far less invasive — requiring only very small incisions hidden within the hairline — and can make a significant difference for patients who are already satisfied with their forehead bone structure.
What changes
Before · Position
At or below the orbital rim — heavy, closed look
After · Position
Above the rim — elevated, open, expressive
Before · Arch
Flat and horizontal — masculine reading
After · Arch
Gently curved — soft, naturally feminine arc
The perception of the eyes as feminine or masculine is not in the eyes themselves — it is entirely determined by the bone and soft tissue frame surrounding them.

Brow Lift
Actual Patient
Combined with forehead surgery
or performed alone
The approach, incision and recovery differ significantly depending on whether the brow lift is part of a full forehead feminization procedure or done as a standalone surgery.
Most common
Combined with Forehead Surgery
When performed alongside forehead contouring, the brow lift uses the same hairline or coronal incision. After the frontal bone is reshaped, the soft tissues of the forehead are lifted and repositioned — adapting the eyebrows to the new bone outline. Without this step, the tissue would fall back over the reshaped bone, or even sag.
Same incision — no additional scarring
Essential for a harmonious result after bone work
1 day hospital stay — shared recovery with forehead
Less invasive
Standalone Brow Lift
When performed alone, the procedure is far less invasive. Very small incisions are concealed within the hair, leaving virtually invisible scars. The hospital stay is half a day, recovery is faster and the result can be very impactful for the right candidate.
Minimal incisions — hidden within the hairline
Half-day hospital stay — faster recovery
Ideal for patients satisfied with their forehead bone structure
What to expect after surgery
Recovery is generally comfortable. The main variable is whether the brow lift was performed alone or alongside forehead bone work — the latter requires more time.
24h
First 24 hours
Rest at the hotel. Head elevated. Avoid abrupt head movements. Bandage in place.
Day 2
Bandage removed
Head can be washed. Cold compresses continue to reduce swelling.
10d
Standalone only
Return to work and social activities. Sutures dissolve on their own — no removal needed.
15-20d
Combined
Return to activities when done alongside forehead recontouring.
Possible risks
Numbness or feeling of tightness in forehead or scalp
Temporary — can last days to several months depending on technique.
Hair loss near incision
Mild and temporary thinning around the incision. Usually resolves fully.
Brow position asymmetry
Correctable after full healing. Or a surprised expression if elevated too high.
Seroma
Occasional fluid accumulation beneath the skin — easily drained if needed.
