Your bones are the key to facial gender
Any person can tell whether a face is masculine or feminine just by looking — and this ability develops in early childhood, even before understanding other anatomical differences. Yet most people couldn't explain what exactly makes a face look female or male. The answer lies in what we cannot see: the hard structure (bones and cartilage) that constitute the forehead, nose, jaw and chin.
The shapes and profile we perceive are determined entirely by these deeper structures. FFS surgery addresses those structures directly — reshaping the facial skeleton to give it feminine proportions. The concept was introduced in the early 1980s by Dr. Douglas Ousterhout, who applied craniofacial surgical techniques to facial gender surgery for the first time.
Since then, Facial Feminization Surgery has become the most comprehensive and effective approach to aligning a person's appearance with their gender — and T-Change has been performing it since 2006.

Eight procedures,
one transformation
Each procedure addresses specific structures of the face. They can be performed individually or combined in a single session for comprehensive feminization.
Forehead Feminization
Contouring and reduction of the brow ridge, frontal sinus and orbital rims — the most impactful single procedure in FFS, transforming the gaze and the way the eyes are perceived.
Feminizing Rhinoplasty
Refining the nose to create softer proportions, a more delicate tip and a straight or subtly curved-in nasal bridge — in harmony with the rest of the feminized face.
Jaw Feminization
Tapering and softening of the jawline through bone reduction — creating the tapered facial silhouette characteristic of feminine faces.
Chin Feminization
Reduction, reshaping or repositioning of the chin to achieve a narrower, generally shorter and pointed lower facial contour.
Cheek Augmentation
Augmentation of the malar region to create softer, higher cheekbones — through implants or fat grafting.
Brow Lift
Elevating and arching the eyebrows to a naturally feminine position — creating a more open and expressive eye area.
Upper Lip Lift
Shortening the philtrum to reveal more of the upper teeth — a subtle but transformative procedure for mid third facial feminization.
Tracheal Shave
Surgical reduction of the thyroid cartilage prominence — achievable through a minimal incision on top of the neck or as scarless endoscopic tracheal shave with incision inside the mouth.
Performing FFS since 2006 —
when few others did
T-Change began performing Facial Feminization Surgery in 2006, at a time when very few surgical teams in the world had the craniofacial training necessary to perform these procedures safely and with lasting results. Currently led by Dr. Javier Rossi — Chief of Head & Neck Surgery at Hospital Militar Central and Professor at University of Buenos Aires — our team has built nearly two decades of focused experience in facial gender surgery.
That experience informs every surgical plan. We know that not every patient needs a full FFS. Sometimes forehead and nose alone are enough to produce a profound, life-changing result — because the forehead affects something crucial: the way the eyes look. The best FFS surgeons know when less is more, and have the confidence to say so.
